- Enter your fax number if you have one. If you do not have a dedicated fax number simply enter the fax number 582 207 8333 which is Petitarian’s VetX dedicated default fax number which will be utilized on all of your Official Virtual Prescription forms issued. We do not disclose your personal information to protect your privacy.
- Please upload a clear copy of your VETERINARY LICENSE. We will verify your license prior to approval to become a Virtual VetX Veterinarian
- Please upload your Valid DRIVER LICENSE. Your Driver License is needed to verify your Veterinarian license.
- Please upload your updated CV/RESUME. This information is for Petitarians use only and will not be shared with anyone.
- Your official RECENT HEADSHOT is required to enable Pet Owners to familiarize themselves with you and to select the Veterinarian with whom they wish to have their consultation calls. Your profile photo will be shown to your clients when they select you as their Vet to call or book an appointment with you. Please smile and provide a one-time color photo headshot similar in content and size to that on your driver license or passport that you want to appear on your VetX personal profile. Your official headshot photo is securely encrypted for your security and is only accessible via your VetX account. Your face must not be blurry and clearly visible. Avoid surgical masks. No AI-generated photos are permitted.
- Write a short bio highlighting your interest, passion, knowledge, skills, and experience as a praciticing Veteranian. This information along with your photo will give th Pet Ownera a good understanding of your abilities and allow them to choose the correct veterinarian for thier needs.
- Choose one or more consultation services you are available to offer. Video Calls, Audio Calls, Booked appointments, Chat Calls. Recommended best are Video Consultations Calls which allow the veterinarian and pet owner to interact directly by seeing and hearing each other. The veterinarian can see the pet and vice versa to better observe and diagnose the pet's condition, diagnose and recommend a treatment plan, and issue prescriptions if required.
- Your payouts are automatically paid daily, weekly, or monthly. Bank name, routing number, and account number are required for ensuring secure deposits to your bank account. Petitarians Vetx uses Stripe as is payout processing gateway provider which requires the last four digits of your Social Security Number for verification.
- Petitarians VetX uses the Stripe payout gateway to enable timely payout to veterinarians. Stripe requires requires a verifiable Employer Identification Number (EIN) or Social Security Number (SSN) to use their payout gateway services; individuals without an EIN can use their SSN or Individual Taxpayer Identification Number (ITIN) instead. Alternatively you may use the last four digits of your Social Security number for your Stripe payment gateway account verification
- Please select your preferred PAYOUT MODE. Payouts can be made to your account either daily , weekly, monthly.
- For your protection and security: All you personal information including your address, telephone number, and fax number will be kept strictly confidential, and only Petitarian’s VeX contact information will appear on all your virtual prescription forms to be presented to the prescription fulfilling pharmacy.

- By checking the box you agree that your e-signature will be used as your Official Virtul Prescription Signature on all your issued virtual prescription forms.
